As an out-of-network provider, we offer reasonable prices for services and kindly ask for payment upfront. Sales receipts are provided. Check with your health plan to see if you’re covered for out-of-network providers.

Do I need a physician's referral to seek services with Live Your Life?

You do not need a physician's referral. However, if you are seeking insurance covered services which you plan to seek reimbursement, the insurance provider may ask for a referral.

How long does each physical therapy session last?

Through our evidence-based and outcome driven approach, we achieve positive outcomes by providing a high quality of care through one-on-one 60 minute sessions conducted by a highly skilled and experienced physical therapist. We have given clients and patients the confidence, the freedom and the ease of mobility to live the type of life they never thought possible.

What should I wear?

Wear comfortable, loose clothing and athletic shoes. During physical therapy sessions, it is important to have access to the injured area for effective treatment. With wearing loose fitting clothing, you can expose the area that we will be examining and treating. For example, if you have a knee problem, it is best to wear shorts. If your problem is with your upper back, shoulders, or your cervical spine, you should wear a t-shirt or tank top.

What are my payment options?

Payment is requiredat the time of services and is covered by self-pay. We accept cash, checks and major credit cards.
