Live Your Life is seeking a highly experienced Florida-licensed physical therapist to provide personalized, one-on-one care in clients’ homes and communities throughout Palm Beach County.
Our private-pay model gives you time to use your clinical judgment, treat the whole person, and build meaningful relationships with clients and families. This is not a high-volume clinic or traditional home health model.
Live Your Life manages intake, scheduling coordination, billing, and payment collection so you can focus more of your time on client care.
We are looking for a highly experienced PT who values clinical independence, prevention, wellness, healthy aging, and relationship-based care.

A Structured Mobile PT Opportunity
As a W-2 per diem physical therapist, you will:
- Work as a W-2 employee in a structured per diem role
- Complete five scheduled client visits on days worked
- Travel to clients throughout Palm Beach County
- Receive mileage reimbursement at the current federal mileage rate
- Have office support for intake, scheduling coordination, billing, and payment collection
Clients may be located throughout Palm Beach County, including Boca Raton, Delray Beach, Boynton Beach, Lake Worth, Wellington, Palm Beach, and surrounding communities.

What You’ll Do
- Perform physical therapy evaluations in clients’ homes and communities
- Develop individualized plans based on each client’s goals, health status, environment, and functional needs
- Provide personalized, one-on-one physical therapy
- Address strength, balance, mobility, endurance, safety, and function
- Educate clients, families, and caregivers
- Complete timely documentation in Jane
Required Qualifications
- Doctor of Physical Therapy (DPT) degree from an accredited physical therapy program
- Current Florida physical therapist license in good standing
- At least 10 years of experience working with the geriatric population
- Current American Physical Therapy Association membership
- Home-based or mobile-care experience
- Current CPR certification
- Strong independent clinical judgment
- Reliable transportation
Board certification as a Geriatric, Neurologic, Orthopaedic, or other relevant Clinical Specialist is preferred.
