Live Your Life is looking for an experienced, Florida-licensed physical therapist to provide personalized care in clients’ homes and communities throughout Palm Beach County.
We are an established mobile healthcare company with extensive experience delivering rehabilitation, prevention, and wellness services outside a traditional clinic. Our Florida practice operates on a private-pay model, allowing physical therapists to focus on thoughtful patient care without insurance authorizations, unit-based billing, or the pressure of treating multiple patients at once.
This W-2 opportunity may be a good fit if you want a consistent mobile caseload, a structured daily schedule, mileage reimbursement, and office support for the administrative side of patient care.
Position Details
Employment arrangement:
W-2 employee
Visit compensation:
- $150 per completed 90-minute initial evaluation
- $85 per completed 60-minute treatment visit
Schedule and service area:
Complete five scheduled patient visits per day
Travel to clients throughout Palm Beach County
Serve patients in communities such as Boca Raton, Delray Beach, Boynton Beach, Lake Worth, Wellington, Palm Beach, and surrounding areas
Receive mileage reimbursement at the current federal mileage rate
Administrative support:
The Live Your Life office handles scheduling, intake, billing, and payment collection so you can focus on clinical care.
Documentation:
Visit documentation is completed in Jane according to company standards.

Clients You May Serve
Your caseload may include active older adults and other adults who need support with:
Balance and fall-risk concerns
Weakness, deconditioning, or gait limitations
Orthopedic conditions
Neurological or functional mobility challenges
Preparation for or recovery from surgery
Strength, wellness, and long-term independence
What You’ll Do
Complete five scheduled patient visits per day
Perform comprehensive physical therapy evaluations in clients’ homes or communities
Develop individualized plans of care and provide one-on-one treatment
Help clients improve strength, balance, mobility, safety, and independence
Create practical home programs and educate clients and caregivers
Communicate with families, referral sources, and other care-team members as appropriate
Complete accurate and timely documentation in Jane
Travel to patient locations throughout Palm Beach County
Required Qualifications
Degree in physical therapy from an accredited program
Current Florida physical therapist license in good standing
At least 10 years of experience working with the geriatric population
Current American Physical Therapy Association membership
Home-based or mobile-care experience
Current CPR certification
Ability to work independently and use electronic documentation
Reliable transportation for travel throughout Palm Beach County
Board certification as a Geriatric, Neurologic, Orthopaedic, or other relevant Clinical Specialist is preferred.
Cancellations
When a client cancels less than 24 hours before a scheduled visit, you will be compensated at the applicable visit rate. A visit canceled more than 24 hours in advance is not paid at the completed-visit rate.
